# 2023 State of the Field - FOCUSED ULTRASOUND FOUNDATION

## 内容

米国[Focused Ultrasound Foundation（集束超音波財団）](https://www.fusfoundation.org/)より、集束超音波技術に関する現状をまとめた報告書「2023 State of the Field」が発表されました。この報告書では、脳神経領域以外や非臨床も含む、さまざまな集束超音波技術の研究や臨床利用について述べられています。

現在、集束超音波技術は166の疾患や障害に対して、さまざまなステージ（多くは非臨床や初期臨床）での開発が行われています。うち32がいずれかの国で薬事承認を得ており、米国では８がFDAから承認されています。2022年、脳神経領域においては双極性障害、転移性脳腫瘍（肺がん）、心的外傷性ストレス症候群（PTSD）、起立性振戦において、初の臨床試験が行われています。

現在本邦はじめ臨床使用が承認されている集束超音波治療は、高エネルギーによる継続的な圧力の波により生み出される熱エネルギーを利用した熱凝固術です。一方で集束超音波は、使用するエネルギーや照射モード（パルス照射等）、併用薬剤（マイクロバブル等）などの組み合わせにより、ニューロモデュレーションやドラッグデリバリーなど新たな治療方法が開発されつつあり、下図（プレビュー２枚目）のように様々なステージで臨床利用や研究が行われています。

集束超音波財団では、集束超音波関連の様々な研究に対する研究費の助成が行われており、その規模は財団予算の６割を越えます。アジアにおいても、東京女子医科大学での頸部ジストニアに対するPallidothalamic tract (PTT) を標的とした凝固術をはじめ、うつ病（MDD）や血液脳関門（BBB）開通など臨床・非臨床の研究が対象となっています。
